Description

If you’ve spent a long time wondering whether what you notice in yourself is real, and whether anyone will take it seriously, this guide is for you. Many people are told it’s anxiety, or stress, or just a busy season of life. Asking for an assessment isn’t dramatic, and you don’t need a diagnosis to be here.

Right to Choose lets you ask your GP to refer you to a provider of your choice, but the process has a lot of moving parts. This guide turns it into six small steps, written from my experience supporting people through it.

You get two files

  • A printable PDF. Print it, tick the boxes, write on the pages, and take them to the surgery.
  • A fillable PDF. The same 12 pages, set up so you can type your notes, tick the boxes and fill in your referral tracker on your computer. It’s a good fit if you’d rather type than write, or if paper tends to get lost.

What’s inside

  • A six-step roadmap you can tick off
  • Choosing a provider: reflection questions on medication, waiting time and what matters to you, with the ADHD UK link
  • Your GP pack checklist
  • “Putting your story into words”: prompts for childhood examples, masking and how it affects daily life, for the days you go blank in appointments
  • Going to your surgery: appointment or reception, and scripts for what to say if your GP or receptionist seems unsure
  • A referral tracker, and what to do if you haven’t heard back
  • What happens next: the ADHD and autism pathways, and what to do about the informant questionnaire if you have no family contact
  • Help with the ADHD questionnaire (ASRS and DIVA), with examples to jog your memory
  • Help with the AQ10 for autism, including why some of the questions don’t fit everyone’s experience, and how to think about details and social cues
  • What to do if your GP won’t agree to shared care for ADHD medication, with a template letter link

This is for you if

  • You’re an adult in England who suspects or knows you’re ADHD, autistic or both
  • You’re self-identified or late-identified, and don’t have a formal diagnosis
  • You feel overwhelmed by the number of steps involved

Good to know

  • 12-page PDF, with two files included
  • Save your own copy of the fillable version after you’ve filled it in. It works best in a desktop PDF reader such as Adobe Acrobat or Preview, and some phone and browser viewers don’t allow typing or saving.
  • Written for England only. Right to Choose works differently in Scotland, Wales and Northern Ireland.
  • Providers and local NHS policies vary and change, so always follow your provider’s instructions.
  • This is not an assessment, and it’s general information rather than medical or legal advice.
  • Already a coaching client? This guide is included, so just get in touch.
